In this case it was a failure to respond to the required activation email. I manually approved the account. Welcome aboard.

Steve, I just went back to your "Have you been banned" post in the support forum, and bolded the part about the confirmation email. That is a crucial part of the process. (And folks should probably be sure their spam filters aren't blocking that confirmation email too.)
