I've replaced them when the heads are worn enough that you have to pound the next smaller size socket on to take them out. It also doesn't help that most people here weld a bar to each bolt head trying to keep them from coming loose.

Actually the strap or bar trick helps protect the hex head from abrasion from rocks etc.
Just torch and grind the weld and the socket goes, that is if the weld is on top the bolt head.
