anyone have any good form letters or resources to find a professionally written request for contract extension?

I thought well a simple INTERNET search should produce what you want and soon learned...depending on who you are requesting this of they may have a form they require you to use.
But I did a source of general forms but of course for sale. You may find a good CD of such forms at a local office supply store. Or if you have a simple legal program may have it.

What type of work are you engaged in? why do you need a time extension? have a look at your contract with your client and see what grounds an extension may be granted.Asking for an extension may leave you open for penalties so examine your original contract very CAREFULLY.
