Operator needs a change of underwear

Came back to the shop on friday afternoon
from what i can find out
was moving concrete pid lids with the forks on the 4in one;
somthing heavy fell back, crushing precleaner and exaust and destroying window
nobody hurt.

The question is - Why did they lift the covers so high? Even to load a cover onto a truck, using forks...... I can't figure how the load fell onto the bonnet?
Only time I have seen damage like that is when the anti-roll back feature is not working on the machine & the operator 'forgets' & rolls the bucket back too far in the fully raised position, & then a rock or piece of concrete rolls out over the back & onto the bonnet/windscreen ........

& those pics are really bad -did the operator take them? & still shaking ? :D
